Current Passport
Current Permanent Resident Alien Card

     2. Copy of Parent or Guardian's Driver's License or Photo ID

3. Primary Proof of Residency (please upload 1 of the following):
Current Mortgage Statement
Current Lease (signed & dated by both landlord and tenant)
4. Secondary Proof of Residency (please upload 2 of the following):
Current utility bill (phone, internet, electricity)
Bank statement
Voter registration card

5. Health Assessment/Immunization Record
Medical clearance must be obtained by the appropriate school nurse before your child can attend school. All new students must show proof of immunizations and a recent physical examination.
