Dear Parents and Guardians,


Plainville Community Schools is changing the medications it stocks in its health offices and will no longer provide acetaminophen, ibuprofen, Tums or Pepto Bismol to students during the school day. This change will take place at the beginning of the 2019-2020 school year. 


If your child needs access to any of these medications, or to any other medication while in school, please follow the instructions below.


In compliance with Connecticut State Law and regulations, the Plainville Board of Education will require a medication authorization form to be completed by an authorized prescriber and signed by the parent/guardian in order for a prescription or non-prescription medication to be administered in school. Please use the following link to access this medication authorization form.  The form can also be found on the district website, www.plainvilleschools.org, from the Health Services Department menu.

Once the form is filled out by an authorized prescriber, please bring it along with the medication to your child’s school.  Medication must be in the original container or a pharmacy prepared container and brought to the school nurse by the parent/guardian or responsible adult. It must be labeled with the name of the child, the name of the drug, strength, dosage, and frequency, along with the authorized prescriber’s name. 

 

The Plainville Board of Education permits students to self carry/administer nonprescription and/or prescribed medication, such as rescue asthma inhalers, insulin and epi-pens for medically-diagnosed allergies, but excluding controlled drugs, provided: (a) an authorized prescriber provides a written medication order for self-administration; (b) a parent/guardian or eligible student over the age of 18 provides written authorization for self-administration of medications; (c) a school nurse has assessed the student’s competency for self-administration in the school setting and deemed it to be safe and appropriate.
